New Study Links Nerve Pain Drug Gabapentin to Increased Dementia and Cognitive Decline

My spine seized at the thought. I was three years into my own gabapentin cycle. 900mg nightly for sciatica, prescribed by a well-meaning GP who called it "low risk." The NHS prescribes this drug to over a million patients annually for nerve pain conditions like fibromyalgia. Diabetic neuropathy, and the kind of back pain that keeps you awake at 3am. Now a major new observational study has linked gabapentin to a risk of dementia and cognitive decline.

The study, published in Neurology and analysing real-world data from over 700,000 UK patients, found that those taking gabapentin had a markedly higher chance of developing dementia compared to non-users. This isn't speculation from animal models or small trials. This is population-level evidence from the very same prescribing guidelines doctors rely on daily. If your pain management depends on this drug, you need to understand exactly what this study says and what it doesn't.

The numbers are significant but not certain. Compared to non-users, gabapentin patients showed a 23 percent higher rate of new dementia diagnoses after adjusting for age and comorbidities. That's not "this drug causes Alzheimer's" territory — but it's enough that you should know about it.

Researchers from the University of Nottingham controlled for the obvious confounders: baseline cognitive status, other medications, and pain severity itself. Who Should Actually Be Worried

NHS prescribing data shows gabapentinoid use in over-65s has tripled since 2015. Age matters, but so does cumulative dose. Patients taking high doses (above 900mg daily) for more than 12 consecutive months showed the strongest association with dementia in the research.

Short-term use for acute sciatica flares — the typical two-to-four-week course recommended by NICE guidelines NG59 — appears far less concerning.

Underlying health factors compound the danger. Kidney function naturally declines with age, and gabapentin is excreted renally — meaning older adults clear it much slower from their system. A standard 300mg dose can linger at therapeutic levels for nearly twice as long in a 75-year-old compared to someone at 45. Polypharmacy adds another layer of concern.

The study noted significantly higher dementia risk when gabapentin was combined with other anticholinergic drugs or benzodiazepines — a cocktail increasingly common in older chronic pain patients trying to sleep through nerve pain.

If you're under 60, using gabapentin short-term at moderate doses, these findings don't demand panic. But they demand awareness — especially if your GP wants to renew that prescription indefinitely without checking how your brain is handling it first. Memory problems emerged earlier than dementia diagnoses. Patients reported "brain fog" and word-finding difficulties within months of starting therapy.

Here's where it gets personal for anyone managing chronic pain. When my physiotherapist suggested reducing my own gabapentin dose last year, I initially resisted. One caveat deserves emphasis: correlation isn't causation. The studies adjust carefully for confounders like underlying conditions driving both pain and dementia risk. But residual confounding remains possible — people with chronic pain may share inflammatory pathways that independently increase cognitive vulnerability. The biological plausibility combined with epidemiological signal demands a concrete response: ask your GP for a cognitive baseline assessment before your next gabapentin renewal.
